Should I continue to conduct discharge surveys?

Short-term rehab units in life plan communities have spent a majority of the last two years being anything but that. Often the “go-to” location to isolate COVID-19 patients, many communities had to discontinue their short-term rehab services. Now, after many months these units can once again function in the essential way they were designed; as a place to support patients with the much-needed services to recuperate and rehabilitate after a surgery, an illness, or an accident.

A discharge survey, administered onsite, just prior to discharge or sent a few days after the patient has returned home can provide valuable information to communities. From preventing re-admission by measuring the effectiveness of discharge orders to assessing the quality of care and attention a resident receives from your staff, a discharge survey solicits that information in an anonymous way.

How were the meals? Did they receive the support they needed both physically and mentally? Were the discharge instructions clear and understood?
